Overview

This study investigates the shear bond strength between calcium silicate-based biomaterials and various glass ionomer cements, essential for reducing bacterial microleakage in vital pulp therapy. A total of 270 acrylic blocks with cavities were filled with three types of calcium silicate biomaterials (ProRoot MTA, Medcem Pure Portland Cement, and Medcem MTA) and subsequently restored using different glass ionomer materials, both with and without adhesive agents. The shear bond strengths were measured after incubation, and statistical analysis was performed using ANOVA and posthoc Tukey tests.

The results indicated that the combination of Medcem MTA with ACTIVA bioactive restorative material, when adhesive was applied, exhibited the highest shear bond strength. Conversely, the ProRoot MTA White paired with Equia Forte HT Fil without adhesive showed the lowest bond strength (P < 0.05). The findings suggest that ACTIVA Bioactive Restorative is a promising option for use with calcium silicate-based biomaterials in vital pulp treatments, and the application of adhesives can effectively mitigate technical challenges associated with immediate restorative placements.

Introduction

The introduction outlines the critical role of pulp treatment in preserving the structural integrity and vitality of dental tissues, particularly in cases affected by caries or trauma. Vital pulp therapy (VPT) has emerged as a key approach in contemporary dentistry for maintaining healthy pulp tissue, especially in situations involving deep dentin caries and pulp exposure. The success of VPT is contingent upon the use of appropriate materials and the establishment of a hermetic seal, with strong shear bond strength (SBS) between biomaterials and restorative materials being essential to reduce bacterial microleakage and enhance long-term outcomes.

The literature indicates that while Proroot MTA demonstrates superior clinical performance compared to various calcium silicate-based materials like Biodentine, it may not excel in all aspects. Limited research exists on the SBS of newer materials such as Medcem MTA and Medcem Pure Portland Cement, which are designed to meet established MTA standards and are claimed to possess exceptional mechanical properties. This study aims to fill the gap by comparing the SBS of these calcium silicate-based biomaterials used in VPT with that of glass ionomer restorative materials, while also evaluating the influence of adhesive application on SBS. Such comparisons are particularly relevant in pediatric dentistry, where procedural efficiency is paramount.

Methods

The study employed three calcium silicate-based biomaterials (WMTA, MMTA, MPPC) and three glass ionomer restorative materials (Equia Forte HT Fil, Fuji II LC, Activa Bioactive-Restorative), alongside a self-etch adhesive (Clearfil Se Bond). A total of 270 acrylic blocks with cylindrical cavities were prepared, with 90 specimens allocated to each biomaterial. The sample size was determined based on an effect size of 0.25, achieving a statistical power of 94.8% and a significance level of 5%. The calcium silicate materials were placed in the cavities, covered with moist cotton pellets, and allowed to harden in a humid incubator at 37 °C for 4 hours. After hardening, surface polishing was performed, and the samples were randomly assigned to groups for the application of glass ionomer restorative materials.

Two bonding methods were employed for the glass ionomer materials: direct bonding without adhesive and bonding with the application of Clearfil SE Bond. After a 24-hour incubation at 37 °C and 100% humidity, adhesive strength was measured using a universal testing machine, with results expressed in megapascals (MPa). Fracture patterns were analyzed using Scanning Electron Microscopy (SEM) after dehydration and drying of the samples. Data analysis was conducted using Microsoft Excel and SPSS, with one-way ANOVA for statistical evaluation and post-hoc Tukey tests for significant differences, defined at P < 0.05.

Results

In the evaluation of shear bond strength (SBS) among various restorative materials, the ABR group exhibited the highest average SBS value, significantly differing from the EFHF and FIILC groups (P < 0.05). Conversely, when comparing MMTA with glass ionomer-containing materials, the EFHF group also showed the highest average SBS (P < 0.05), with no significant difference observed between the FIILC and ABR groups (P > 0.05). For MPPC, no significant differences were found among the groups (P > 0.05), with the FIILC group recording the highest average SBS of 10.29 ± 11.64 MPa, while the EFHF group had the lowest at 6.36 ± 4.95 MPa.

Among non-adhesive applications, the MMTA + EFHF combination yielded the highest average SBS of 21.48 ± 8.95 MPa, whereas the WMTA + EFHF group had the lowest at 4.94 ± 2.13 MPa. The WMTA + FIILC group also recorded the lowest average SBS among adhesive applications, while the ABR group achieved the highest. Statistical comparisons between adhesive and non-adhesive groups for the same biomaterial-restorative combinations are detailed in Tables 2, 3, and 4. Fracture analysis revealed cohesive failure as the predominant type across all groups (P < 0.05), with the MMTA group exhibiting the highest frequency of fractures and the MPPC group the lowest. Although mixed fractures were most prevalent in the MPPC group, this finding was not statistically significant (P > 0.05). SEM micrographs illustrating common fracture patterns at 1000x magnification are provided in Figures 2, 3, and 4.

Discussion

In the discussion of vital pulp treatment (VPT), the importance of achieving stable pulpal hemodynamics and hermetic coronal restoration is emphasized to prevent severe inflammatory reactions. The ideal biomaterial for VPT should stimulate dentin formation and resist bacterial infiltration, making the bond between restorative and biomaterials critical. The study compares the shear bond strength (SBS) of various calcium silicate-based biomaterials—white mineral trioxide aggregate (WMTA), modified mineral trioxide aggregate (MMTA), and mineral trioxide aggregate-based pulp capping material (MPPC)—with restorative materials, revealing that WMTA exhibited the lowest average SBS values without adhesive application. Notably, the application of adhesive agents significantly improved bond strength, particularly with Activa Bioactive Material (ABR), which demonstrated superior SBS values across various combinations.

The findings indicate that while the bond strength of restorative materials to calcium silicate-based biomaterials varies significantly, the use of adhesives enhances this bond, particularly in the ABR group. The study also highlights the need for further research to explore the effects of saliva contamination and the completion of setting reactions in clinical scenarios, as in vitro results may not fully replicate clinical behavior. The results suggest that achieving a bond strength of 17-20 MPa is essential for effective sealing and resistance to contraction forces, with only specific combinations meeting this criterion. Overall, the study provides valuable insights for future investigations into the performance of these materials in pediatric dentistry.
