Overview

This study aimed to assess the impact of short dentin etching and 6-month water storage on the microtensile bond strength (µTBS) of universal and two-step self-etch adhesives to dentin. Dentin specimens from 56 third molars were divided into two groups based on adhesive type: universal adhesive (Scotchbond Universal Plus) and two-step self-etch adhesive (Clearfil SE Bond). Each group was further subdivided according to the application protocol—self-etch (SE) and etch-and-rinse with short dentin etching (E&R/SDE) for 3 seconds. After bonding, specimens were stored either for 24 hours or 6 months before being tested for tensile strength.

Results indicated that the application protocol and storage duration significantly affected µTBS (p < 0.001). Specifically, E&R/SDE for 3 seconds resulted in higher bond strength compared to SE for both adhesive types (p < 0.05). However, all groups experienced a significant reduction in bond strength after 6 months of water storage (p < 0.05), with no notable differences between the adhesives across protocols or storage times (p > 0.05). The predominant failure mode shifted from mixed failure in immediate groups to adhesive failure after 6 months. These findings suggest that while E&R/SDE enhances immediate bonding performance, long-term durability remains a critical issue, warranting further clinical investigation.

Methods

In this study, two types of dental adhesives were utilized: a universal adhesive (Scotchbond Universal Plus Adhesive, 3M Oral Care, St. Paul, MN, USA) and a two-step self-etch adhesive (Clearfil SE, Kuraray Noritake Dental Inc., Okayama, Japan). Both adhesives were paired with a nanohybrid resin composite (Neo Spectra ST LV, Dentsply Sirona GmbH, Konstanz, Germany). Detailed descriptions of the materials and their handling procedures are provided in Table 1.

Photo-polymerization of the adhesives was conducted using a light-emitting diode (LED) curing unit (Elipar S10, 3M Oral Care), which emitted light at a wavelength range of 430-480 nm and operated at a power intensity of 1200 mW/cm². The curing unit’s performance was verified using a built-in radiometer to ensure proper polymerization conditions.

Results

The results of the study indicate that the mean bond strength values (µTBS) varied across different groups, as summarized in Table 2. A 3-way ANOVA analysis demonstrated that the type of adhesive did not significantly influence bond strength (p = 0.06). In contrast, both the application protocol and storage time had a significant impact on bond strength values (p < 0.001). Specifically, for immediately tested groups, Tukey HSD multiple comparison tests indicated no significant differences in bond strength between Scotchbond Universal Plus and Clearfil SE adhesives when applied using either the SE protocol (p = 0.819) or the E&R/SDE protocol (p = 0.952). However, etching the dentin significantly enhanced bond strength for both adhesives compared to non-etched groups (p < 0.05). For aged groups, no significant differences were observed between the two adhesives in either the SE protocol (p = 0.980) or the E&R/SDE protocol (p = 0.999). Nonetheless, dentin etching again resulted in significantly improved bond strength compared to non-etched groups (p < 0.05). After 6 months of water storage, all groups exhibited a significant reduction in dentin bond strength compared to immediate testing (p < 0.05). The failure patterns, depicted in Fig. 2, showed that mixed failures were predominant in immediate groups, while adhesive failures were more common in aged groups. Representative images of these failure modes are provided in Fig. 3.

Discussion

The discussion section of the study highlights the challenges and findings related to adhesive bonding in dentistry, particularly focusing on the bond strength of two adhesives: Scotchbond Universal Plus and Clearfil SE Bond. The study, conducted with ethically sourced human third molars, aimed to evaluate the microtensile bond strength (µTBS) to dentin under varying application protocols and aging conditions. Results indicated that while both adhesives exhibited higher µTBS when using the experimental etch-and-rinse/simplified dentin-etching (E&R/SDE) protocol compared to the self-etch (SE) mode, significant bond strength deterioration was observed after six months of water storage. This decline was attributed to hydrolysis affecting the adhesive interface and the hybrid layer.

The findings also revealed that the Scotchbond Universal Plus adhesive performed comparably to the Clearfil SE Bond, despite differences in their chemical compositions. The study emphasized the importance of the etching protocol, noting that the E&R/SDE method improved bonding by effectively removing the smear layer without excessive demineralization of dentin. However, the long-term durability of the bond remains a concern, as evidenced by the increased incidence of adhesive failures after aging. Limitations of the study, including the short etching time and the in-vitro nature of the experiments, suggest the need for further research to explore a wider range of adhesives and more clinically relevant conditions. Overall, while the E&R/SDE protocol shows promise for enhancing immediate bonding, the long-term implications necessitate cautious interpretation in clinical practice.
